The Oglala Sioux Lakota Nursing Home (OSLNH) is a state-of-the-art, 60-bed Long-Term Care and 12-bed Memory Care facility, located south of Whiteclay, Nebraska. Owned by the Oglala Sioux Tribe and managed by Native American Health Management (NAHM), we are dedicated to providing high-quality care in a culturally sensitive environment that honors and respects the traditions of the Lakota people.
For over 15 years, the Oglala Sioux Tribe envisioned a place where Lakota elders could receive quality healthcare close to home. Today, OSLNH is proud to fulfill that vision by offering skilled nursing services, memory care, and a welcoming environment that celebrates Lakota culture, language, and traditions.
Our Services
Long-Term Care
Our Long-Term Care Unit, established in 2016, provides 24-hour nursing care, restorative therapy, and assistance with daily living activities. We ensure our residents maintain dignity, comfort, and a strong connection to their community.
Memory Care Unit
Opened in June 2020, our 12-bed Memory Care Unit specializes in Alzheimer’s and dementia care. It offers structured routines, enhanced safety, and engaging activities designed to support cognitive function and emotional well-being.
Culturally-Centered Activities
OSLNH fosters a home-like environment where residents can participate in Lakota cultural activities and traditional crafts. Our Activities Department provides group and individualized programs, including:

Become a Resident
We are currently accepting admissions for both Long-Term Care and Memory Care. Eligibility is open to enrolled members of federally recognized tribes and their spouses. Our Social Services team can assist with Medicaid applications and transition planning.
